Did
you know that you could be held responsible for copyright violations
on your packets? Publishers are prosecuting professors, universities,
and duplicating businesses.
Most of the following documents are in the portable document format (pdf) and
require Adobe Acrobat Reader.
Course
Packet Information Brochure: This brochure is to be
used to help you prepare your course packets. It describes the copyright
permissions process that Quick Copy Centers provides. The forms that are
required for submitting your course packet to Quick Copy Center are included
in this brochure.
Course
Packet Request Form: This is the main form used in
getting copyright permissions. It lists the information that we need
in Quick Copy Centers in order to get copyright permission. One set is
included in the Course Packet Information Brochure.
Indemnification
Form: This is a form that must be completed and sent
in with any course packet containing any non-copyrighted information.
It is a supplement to the Course Packet Request Form.
Quick
Copy Job Information Form: This form is required
along with the Course Packet Form and Indemnification Form.
Professor
Royalty Form: This is the permission form for an
original copyrighted work for a faculty member.

Information
The Quick Copy Centers offer assistance in obtaining copyright clearances for
course packets. With the support of UT System’s Attorney and Georgia Harper’s
guidelines, we are following these guidelines to protect our faculty, the University
of Texas and the Quick Copy Centers.
Copyright Brochure
The Course Packet Information brochure (provided in pdf format to your right)
contains general information on the copyright laws, do’s and don’ts
for copying, a course packet request form, and an indemnification form to help
speed up the copyright approval process.
Course Packet Deadlines
Due to the large number of course packets that Quick Copy Centers handle for
the faculty, we have a four to six-week deadline for all course packets. This
deadline was enacted to ensure that we are able to review all of the course packet
material and be sure the material complies with the fair use and copyright laws.
The deadline also ensures that your course material will be ready by the first
day of class.
Occasionally, we have to obtain permission from an individual copyright holder/publisher,
which often takes several weeks before a response is received. If your course
packet contains copyrighted material, we will not be able to reproduce this packet
for your students until we obtain permission.
All packets turned in after the six-week deadline will be processed on a first-come,
first-serve basis. |
